I was very happy when i found your model. I was looking for a long time something like that from Thingiverse. I sliced it with Cura and used "Touching buildplate" -support. Print angle you can see from the photo :)

Great to hear it was used. I may need to print i5c combination as well. What kind of support did you use? I printed adapter in different direction (laying flat) and attached phone dedicated mount.
